Why one number gets two different answers
A ferritin of 25 is the clearest example in all of blood work of the difference between normal and adequate.
Laboratory reference ranges commonly start at 12 or 15 ng/mL. Those floors come from the statistical distribution of whoever the lab sampled — a population that included plenty of people with undiagnosed iron deficiency. Being inside the range means you are not in the bottom few percent of that group. It does not mean you have enough iron.
Against that, a cutoff of 30 ng/mL is roughly 92% sensitive and 98% specific for iron deficiency. The World Health Organization reserves 15 ng/mL for outright depletion. So there is a wide band — roughly 12 to 30 — where the report says normal and the clinical literature says deficient.

The catch that cuts the other way
Ferritin is an acute-phase reactant: inflammation, infection, liver disease and obesity all raise it regardless of how much iron you have stored. That means a normal ferritin can mask a real deficiency when inflammation is present — the two effects cancel out on the report.
This is why ferritin alone is an incomplete test. Transferrin saturation stays low in deficiency even when inflammation lifts ferritin, and CRP tells you whether inflammation is in play at all. The full iron panel covers both.
What to do next
- Compare against your own history, not just the range. A slide from 70 to 40 to 25 is the finding, even though every one of those was reported as normal.
- Ask for the full panel — serum iron, TIBC, transferrin saturation, plus CRP to check for inflammation.
- Don't self-supplement on a single reading. Iron overload is a real risk and the treatment differs by cause.
- Bring symptoms with you. Fatigue, hair shedding, breathlessness on stairs and restless legs are the ones most often dismissed at a borderline ferritin.
- Talk to a clinician if you are below 30 with symptoms, or above 300 at all.
